	Attached below is the template that should be used when submitting
a bug or reporting anomalies with either mod_fastcgi or Developer's kit.
This is to aid myself and others to better understand the problem and 
answer it to the fullest extent possible.  Just cut and paste the form 
before mailing it to the list and also make the Subject: line of your 
email message start with BUG REPORT:
